You can self-certify if you've been ill for less than a week using this form: 

If you have recently been in a hospital, your consultant can provide you with a fit note; please contact the hospital, who can arrange this for you.

If you have already self-certified and still are unwell to return to work, please make an appointment with a doctor who can assess you and confirm if you require a fit note.
